New Patients
New patients are welcome. All new patients are asked to complete the medical history and patient registration forms online and must present their insurance card(s) and identification upon their first visit to our office. If you are unable to complete the registration online or have any difficulties doing so, you are recommended to arrive at least 15 minutes prior to your appointment to complete the registration in the office. If you prefer to complete the paper registration, you can download and print the forms below. Please remember to bring the completed printed forms to your appointment.
Returning patients are also required to check in online and arrive about 5 minutes prior to their appointment time.
The link to check in and register for all patients will be sent via text or email approximately three (3) days prior to your appointment date.
For those taking medication, we ask that you bring a medication list to the appointment. It is also the patient's responsibility to bring their referral, if applicable, with them to our office.
